Any credit card purchases through OHIMA will appear as TELUSYS on your credit card statement.
Register by MAIL with check or money order.
Print out the REGISTRATION FORM and mail registration(s) with payment to:

Registrations MUST be accompanied with payment. Forms received without payment will not be processed. All registrations/payment must be postmarked no later than FRIDAY, MARCH 11th to qualify for the early bird discount. Checks should be made payable to OHIMA. Purchase orders are not accepted. You may register after 3/11/11 or onsite, but must pay the late fee. An additional $15 processing fee will be added for all onsite registrations. Your cancelled check serves as your receipt, registration confirmation notices will not be sent. OHIMA EIN: 31-0918571
You must provide a valid email address on your registration form to access the speaker materials online. OHIMA does not release member email addresses without permission.
Cancellation Policy
Registration fees will be refunded less a $25 processing fee if notice is received by Monday, March 14, 2011. Cancellations received after this date will not receive a refund, but substitutions are permitted - contact OHIMA.
