2009 OHIMA Exhibit Booth Reservation
You may now make an online reservation to secure your booth for the 2009 OHIMA Trade Show. Booth deposits will be accepted through October 3, 2008. Beginning 10/4/08, full payment is required to reserve booth space.

BOOTH FEES
If signed contract with full payment is postmarked on or before 12-31-08:
Standard: $630.00
Premium (corner booths)*: $655.00
If signed contract with full payment is postmarked after 12-31-08:
Standard: $655.00
Premium (corner booths)*: $680.00
* For 12'x16' Corner Booths (Booth #7 & #54) -add $50
Complete the ONLINE BOOTH RESERVATION FORM indicating your booth preferences. You will receive a confirmation email from OHIMA within 24 hours that your booth reservation has been received.
Reserve your booth by making a deposit:
Mail a $100 check to OHIMA within 10 days of making your booth reservation. If payment is not received, your booth will be released for sale. Please make check payable to OHIMA and mail to: OHIMA, PO Box 824, Worthington, OH 43085-0824.
OR
 Charge your deposit through our secure server via a link on the ONLINE BOOTH RESERVATION FORM. ONLY Visa or MasterCard accepted.
Deposits are non-refundable but may be applied to next year's show.
An invoice will be sent to you for your remaining booth balance along with the 2009 Exhibitor Contract. Your booth balance and signed contract are due by October 17, 2008. If payment is not received, the booth will be released for sale; however, your deposit may be applied towards another booth if your first choice is no longer available.
